Community Mental Health Nurse

37.5 hours per week
Band 06 £25,528 to £34,189 pa plus Outer London Allowances

Richmond CDAT is a joint health and social care, multi-disciplinary team that provides assessment, detoxification, care planning, residential referrals and day programmes.
The service provides information, advice, counselling support for people with drug and/or alcohol problems; home visits and open drop-in sessions; liaison with statutory and voluntary agencies and specialist care within a tier 3-structured community based drug treatment service under Models of Care.

Tier 3 structured services include community detoxification, treatment for stimulant problems, health promotion and harm reduction interventions, community care assessment and care management, community-based aftercare programmes for drug and alcohol misusers leaving residential rehabilitation or prison.

There is close interagency working with GPs, Primary Care and the local voluntary sector services, liaison and partnership working with Adult Mental Health services to meet the needs of drug users with Dual Diagnosis

You will work as an effective member of a multi disciplinary integrated health and social care team; to care co-ordinate support to drug and alcohol users; to communicate and work collaboratively with all agencies and agents involved in supporting drug and alcohol users.
Post qualification experience is essential as is experience of working in the community, knowledge of substance misuse; assessment and care management.
